To view your personal benefit information choose the "Members" page to log in as follows:

Username - Your username is different for all, and consists of the following, all in lowercase. Last initial, first initial, last 4 digits of your SSN, and year of birth (10 characters in total).

Password - Month of birth (2 digits), day of birth (2 digits)

Example:
John Doe, SSN 111-22-3333, DOB 4/5/1966

Username: dj33331966
Password: 0405

The first time you log in, you will be prompted to change your password to something confidential, and known only to you, please do so and proceed.
