As a union member your chiropractic/acupuncture/herbal therapies benefit is provided at no cost to you. If you so desire, you may purchase this benefit for your spouse and children. The benefits and cost of this program are as follows:

All rates are on a per pay period basis. An application must be submitted for all supplemental and voluntary plans. Eligibility requirements may be waived if enrolling within thirty (30) days of employment.
