Voluntary Long-Term Disability Insurance

As A Union Member, you may upgrade the basic long-term disability plan. The voluntary plan offers the following coverage:

60% of monthly pay, up to a maximum of $2,000 per month
Benefit payable as long as you are disabled, up to a maximum age of 65 years
13 week waiting period
Benefit paid may be offset by sick pay

A personal health statement enrollment form is required to determine eligibility. Eligibility requirements may be waived if enrolling within the first thirty (30) days of employment.
